Please follow these instructions:
- Select your draw (then player fields will appear).
- Select you and your partner's name from the player drop down lists. For the Crafty Veterans draw, select just yourself (ignore player 2).
- Fill in the contact information fields (email address and phone number).
- Click the 'Submit Application' button at the bottom of the page.
